In the multidisciplinary team, there are pharmacists, nurses, doctors, and other allied health professions. The different members of the multidisciplinary team have different roles, responsibilities, and skills which they come together to provide the best care to their patients and the community. In the multidisciplinary team, meetings are held where patient’s cases would be discussed how their treatment could be improved. Pharmacists working with a team with other health professionals would be able to apply their own skills together. The doctor’s role is to diagnose the patient’s illness and give them treatment. The role of the nurse is to take care of the patients, make sure the patient has had their medication, and assist the patient and nurses also collect data about the patients. Pharmacists offer advice to patients and the public about their health and how to use the medications safely. A pharmacist also offers advice to nurses and doctors when choosing medications for the patient. The pharmacist also makes sure that the patient does take their medications safely and know how to, they need to also need to make sure that medications are safe to take with other drugs. The pharmacist would discuss with patients how they could benefit from taking the medications which are prescribed to them and also tell them about the side effects that they could have from taking the medications. Pharmacists would assess conditions and make decisions about which medicines the individual should take. Pharmacists will need to have good communication skills when talking to patients and health professions and need to be able to work in a team and need to be accurate with the dosage of the medication.

Pharmacist and pharmacy students have to follow the fitness to practice. There are 9 standards which are leadership, person-centred care, partnership working, effective communication, professional knowledge and skills, professional judgment, professional behaviour, confidentiality and privacy, and speaking up about concerns. These need to be followed by every pharmacist and pharmacy student. The 9 standards are there to be followed so that care towards the patients and community is delivered safely and effectively. As a pharmacist, you must make the patient your priority and provide them with the best suitable medication and provide the patient with a safe effective treatment so the patient’s health would improve. Pharmacists need to have a good relationship with their patients so they could be comfortable and be able to trust you and be honest about their health. As a pharmacist, you must respect the confidentiality and privacy of the person’s personal details by talking with the patient in a private room and only talk about the patient with the relevant people.
